Fatal Accident During Repairing Work Of Conveyor

Description of the Accident

A worker was killed by the pinched effect between the drum and the conveyor belt during repairing work on a conveyor belt (Figure a). On the day of the incident several workers were carrying out works to connect the disjointed belt on a conveyor used to transfer oil palm fibre from storage to the boiler house. At that time, the victim was standing on a disjointed belt when his colleague switched on the conveyor. As a result, the victim fell and trapped between drum and the belt (Figure b and c).

Factors Contributed to the Accident

  • Conveyor was turned on when the repairs were being carried out
  • The victim did not anticipate the consequence of unsafe act
  • The unsafe act of victim was not reprimanded
  • Insufficient skill/knowledge to carry out the repair work

Recommendations of Preventive Action

  • Log Out Tag Out System (LOTO) should be implemented
  • The work should be supervised to ensure that no unsafe act
  • Safety briefing should be given before commencing of work
  • Safe work procedure should be provided and complied
