Summary of Accident:
A member of the public was killed when he was struck by a bulldozer when he went into the plantation area to meet his friend who was working there. During the incident, a bulldozer was uprooting, lifting and moving the oil palms. While the bulldozer was reversing, it fell into a hole, overturned and struck the victim who was looking for his friend. The victim died at the scene, pinned by the bulldozer. He sustained serious bodily injuries.
Contributing Factors :
The victim had entered the work area without obtaining permission from the responsible party and approached a bulldozer which was being operated.
Results of Observation :
Lack of monitoring of outsiders entering the work area and failure of the employer to provide safe work procedures for works carried out at the plantation.
