Collapse Of Concrete Slab And Formwork Structure During Construction

This safety alert is published following an accident at a construction site in Terengganu. An employee was fatally injured when the formwork and temporary propping structures collapsed during concreting work on the upper floor slab. The deceased had fallen to the floor below, together with the formwork and propping structures that suddenly collapsed.
The incident had also caused injuries to another four workers who were working together with the deceased.

From the investigation that had been conducted, it was found that the concreting work for the fourth floor slab was in progress. The concrete was hoisted up by using the mobile crane and unloaded onto the building's upper floor slab. The incident happened during the last pour as per available information.

Legal Requirements
1.
The Occupational Safety and Health Act 1994, requires employer to conduct and identify hazards at the workplace before the risk can be assessed and controlled to a standard as far as practicable. It also requires principal contractors and sub-contractor to provide implement and maintain safe systems of work. The employer is also required to develop and implement safe work method statement for any hazardous construction tasks that have been identified. Such information from the method statement has to be communicated to all the workers who are involved in such work.

The employer has to ensure that all parts of formwork, false work and temporary propping assemblies are stable and secured at all times during the concrete pouring process. The designated person appointed by the employer has to check on the safety of the temporary work structures whilst the concreting work is in progress.
3.
The contractor shall comply with Part III, Factories and Machinery (Building Operations and Work of Engineering Construction) (Safety) Regulations 1986 on concreting work.
