Summary of Accident:
A worker at a construction site was killed after falling from a work platform. During the incident, the victim was carrying out bar bending works for the roof beam at sludge holding tank 4 of the dewatering house together with other workers. While the victim was walking along the work platform to go for a rest, he suddenly lost his balance and fell to the first floor. Consequently, he sustained serious head injuries and died on arrival at the hospital.
Contributing Factors :
While walking on the work platform, the victim lost his balance and fell to the first floor.
Results of Observation :
Lack of safe work procedures.
