Summary of Accident:
A carpenter was killed when he fell from the 23rd floor to the 7th floor. During the incident, the victim was installing the wall formwork. While he was standing on the work platform, one of the brackets of the work platform came loose and the platform became unstable, causing him to fall down. The victim was killed at the scene of the accident owing to the serious injuries he sustained.
Contributing Factors :
One of the brackets of the work platform came off while the victim was standing on top.
Results of Observation :
Lack of safe work procedures and monitoring by the employer in the use of the work platform.
