Summary of Accident:
A construction worker was killed after falling from the 16th to the 6th floor. During the incident, the victim was installing the wall formwork on the 16th floor. While stepping out from the edge of the building onto the work platform, he trod on one end of the platform plank, causing it to break. The victim fell down to the 6th floor and died at the scene.
Contributing Factors :
The victim accidentally stepped on one end of the platform plank, causing him to fall.
Results of Observation :
Lack of safe work procedures and failure of the employer to ensure that the work platform was safe for the passage of the worker.
