Audiometry Test Centre (PUA)

1.0 INTRODUCTION

Occupational Safety & Health (Noise Exposure) regulations 2019 state that every employer has conducted a noise risk assessment and shows that any employee is exposed to excessive noise exceeding the limit specified in the regulations, the employer shall cause an audiometric testing to be carried out on such employee annually. For the purpose of carrying out the audiometric testing, the employer shall appoint an audiometric testing centre approved by Director General.

2.0 APPLICATION PROCEDURES 

  2.1 Application
  1. Form and checklist for Audiometric Test Centre approval application (download here)
  2. Applications must be submitted using letterhead and addressed to:
Director General
Department of Occupational Safety and Health Malaysia
Levels 1,3,4, and 5, Block D4, Complex D,
Federal Government Administrative Centre 62530 Putrajaya
(U / p: Industrial Hygiene and Ergonomics Division)

  2.2 Documents

   2.2.1 Registration procedure
  1. Static audiometric testing centre─
    1. Design drawing and location plan of audiometric test room;
    2. Calibration report of audiometric test room and audiometer by an audiometric calibration service provider;
    3. Training certificate of audiometric technician from supplier or any training provider recognized by DOSH;
    4. OHD registration certificate;
    5. Audiometric testing procedures including format of test results.
    6. Audiometric technician appointment letter; and
    7. OHD appointment letter.
  2. Mobile audiometric testing centre─
    1. Vehicle Ownership Certificate as Mobile Services Clinic from Road and Transport Department;
    2. Audiometric test room design drawing endorsed by vehicle inspection company appointed by Malaysian Government;
    3. Valid calibration report for 'Precision Sound Level Meter Type/Class 1' and 'Sound Calibrator Type/Class 1' by manufacturer or laboratory accredited by Department of Standards Malaysia;;
    4. Format calibration report of audiometric test room and audiometric measuring equipment;
    5. Procedure to conduct measurement on background sound level in audiometric test room and calibration of audiometer;
    6. Training certificate of a technician from supplier or any training provider recognized by DOSH;
    7. Audiometric testing procedures including format of test results.
    8. OHD registration certificate;
    9. Audiometric technician appointment letter; and
    10. OHD appointment letter.

  2.2.2 Registration renewal
    Application for renewal shall be submitted to Director General together with the copy of following documents ─
  1. Calibration report of audiometric test room and audiometer (for static audiometric testing centre);
  2. Latest calibration report for 'Precision Sound Level Meter Type/Class 1', 'Sound Calibrator Type/Class 1' by manufacturer or laboratory accredited by Department of Standards Malaysia (for mobile audiometric testing centre);
  3. Additional related information as following: -
    1. OHD certificate and appointment letter;
    2. audiometric technician appointment letter and his training certificate.

    Application for registration renewal shall be submitted to the Director General at least one (1) month before the expiry date of the current registration.  

3.0 APPROVAL PROCESSES

DOSH will conduct audiometric testing centre facility assessment upon receipt of complete application document. The decision will be issued within 30 working days from the date the complete application is received.
