Summary of Accident:
A worker, who was installing street lights, was killed when the skylift he was using overturned and crashed onto the road. During the incident, the victim was in the bucket and was changing a fluorescent light. The skylift was suddenly uplifted, causing the bucket to smash onto the road. The victim sustained serious injuries and died at the scene.

 
Contributing Factors :
The skylift crashed suddenly onto the road and the victim could not save himself in time.

 
Results of Observation :
Lack of safe work procedures and close monitoring by the employer.
